Indianapolis-based Formstack, a software-as-a-service company that helps businesses manage information and automate workflows through a suite of powerful online form tools, has named Dustin Sapp chief operating officer. He is an award-winning business leader, entrepreneur and technologist. Sapp most recently served as co-founder and chief executive officer of Octiv (formerly TinderBox). He wrote the initial code for its technology and has since led the organization to become a leader in online sales productivity technology. Sapp will continue to be involved with Octiv’s board of directors.

Before Octiv, he served as the president and co-founder of Vontoo, a voice technology platform, and co-founder of NoInk, a mobile sales productivity solution for the medical device industry. Sapp has been named to American Venture Magazine’s Top 40 Under 40 and Indiana Chamber’s Dynamic Leader of the Year. He holds a Bachelor of Science in Computer Engineering from Rose-Hulman Institute of Technology.
