Community Health Network Forms Partnership

Community Health Network Forms Partnership

Community Health Network has signed an agreement with Ohio-based CHI Health at Home for a joint venture partnership to offer home-based health care in central Indiana. The new partnership is expected to enhance services and allow an expansion of home care. 

“We’ve pursued this partnership on behalf of Community Home Health Services because it is an opportunity for Community Health Network to partner with another industry expert to find new ways to grow in all areas of service, infuse strong population health strategies, and maximize new technology to enhance the experience for our patients and our caregivers,” said Community Health Network Chief Financial Officer Kyle Fisher.

Community Health at Home has now joined the system and is providing home care to existing patients while uniting over 200 home health care workers to CHI Health at Home’s electronic medical records system, Homecare Homebase. 

“Community Health Network and CHI Health at Home share common core, organizational, and long-term values for care”, said CHI Health and Home President and CEO Dan Dietz.  “Both organizations will bring their strengths to the table to create the next evolution of home health care services, together”.

Community Health at Home, through AmeriMed, will later this year begin to also provide home infusion services in Central Indiana.
