Diabetes Prevention Program Reaching South Bend
Mishawaka-based Saint Joseph Health System is among five hospital groups that will benefit from a $7 million diabetes prevention program. The effort, funded by a Centers for Disease Control and Prevention grant, involves partnering with Michigan-based Trinity Health on an evidence-based lifestyle change initiative in underserved populations within communities like South Bend.
The program will provide resources, information and connect prediabetic individuals or those at risk for Type 2 diabetes with lifestyle coaches on improved eating habits, stress reduction, weight loss and increased physical activities.
Participating organizations include: Saint Joseph Health System in the South Bend area, Holy Cross Hospital in Florida, Mercy Health in Michigan, Mount Carmel Health System in Ohio, Saint Joseph Mercy Health System in Michigan and St. Peter’s Health Partners in New York.
