Franciscan, CVS Partner For Patient Care

Subscribe NowMishawaka-based Franciscan Health has announced a partnership with Rhode Island-based CVS Health Corp. (NYSE: CVS) that it says will enhance access to affordable health care services. The clinical affiliation agreement aims to improve coordination between Franciscan primary care and CVS MinuteClinic providers.
Franciscan says CVS Health will provide enhanced prescription and visit information to the health system through secured electronic health record systems. Patients will also have access to a variety of services, including clinical support, medication counseling and chronic disease monitoring, at CVS stores and MinuteClinic locations.
The health system says the collaboration will see the partners working together to plan strategies around chronic care and wellness. Patients without regular access to health care will also be able to visit MinuteClinic locations for information on finding a primary care physician.
The use of electronic health record systems will allow CVS to share messages and alerts regarding medication non-adherence issues and, with the patient’s consent, sharing patient visit summaries with the patient’s primary care physician.
Franciscan Chief Executive Officer Kevin Leahy says the partnership will further enhance the overall continuity of care for patients.
